1. 10-13 employees exposed daily to PVC glue fumes. 2. Uncertified employees utilizing forklifts on the floor. 3. Pallet racks are not properly secured, with only one bolt per upright. 4. Employer is not engaging in safety training meetings or demonstrations. 5. There is a lack of ear plugs and other noise hazard safety controls in loud environments. 6. Loose bolts on mezzanine upright supports. 7. Evidence that employees might be smoking indoors, potentially near hazardous chemicals given the nature of the employer. 8. Employer is not enforcing COVID19 PPE requirements, such as facemasks.
