1) The restroom is not sanitary. The toilets are not firmly secured to the floor and are broken. The restrooms are not cleaned daily. Paper products and soap are not replaced to ensure proper handwashing during this COVID-19 pandemic. 2) The heating furnace units are not operating. This has been an ongoing issue for years. 3) Space heaters which are owned by employees are used to heat the store. 4) The space heaters are plugged into extension cords that are going across the floor causing tripping hazards. 5) The extension cords are attached to temporary wiring. 6) The space heaters are running the entire day, and are not turned off at the end of the night. This presented a fire hazard. 7) The store has not developed or implemented an COVID-19 Prevention Program which included guidance to include but not limited to : separating and sending home infected or potentially infected people from the workplace, implementing physical distancing, installing barriers where physical distancing cannot be maintained, and suppressing the spread by using face coverings, providing supplies for good hygiene, and routine cleaning and disinfection of the store.
