(1) The employer does not have a COVID-19 plan in place. Two employees, handling a patient who tested positive for COVID-19, may have been infected by the virus. The employer did not test or quarantine the employees. (2) The ceiling has collapsed in the upstairs' room in the main building. Sheet rock and insulation is hanging from the ceiling and could fall onto an employee. (3) There is an infestation of rodents and roaches throughout the building.
