- comments: observed approximately 15 flying insects in the food prep areas. Observed 5 live house flies and 10 small flying insects on the walls and flying around in the food prep areas. Additional pest control service is needed to eliminate the pest activity. Priority foundation 7-38-020(a). Citation issued.

- comments: observed the door gasket torn around the inside of the 1 door prep cooler. Management instructed to repair and maintain.
64. Public health nuisance
- comments: observed a customer not wearing a face mask while waiting to receive their food. Management instructed that all customers must wear a face mask inside of the restaurant. Citation issued 7-28-060.
