1. Employees are not provided with N95 face masks when working directly with patients. Employees are told to reuse the non-N95 face masks they have been provided or bring their own cloth face coverings from home. Hazard Location: All 5 locations, with emphasis on the Las Vegas

1. The employer is not providing employees with gloves unless patients have an eye infection. 2. Employees are required to touch patients' eyes as part of normal appointments, potentially exposing employees to COVID-19 hazards. 3. Employees are concerned with the harshness of the cleaning chemicals being used.
4. The employer is not providing masks to employees, but is requiring their use. 5. All facilities are low on disinfectant, which has led to the employer diluting the solution with water, potentially exposing employees to COVID-19 hazards. 6. The employer is screening patients for COVID-19 symptoms, but is allowing them entry to the facility even if they are showing symptoms such as coughing. 7. The employer is allowing patients with a positive COVID-19 test result into the facility without first verifying a negative result. Hazard Location: 1. At all Shepard Eye center locations in the Las Vegas and Henderson area 5/22/2020 JH

be only for shipping 6.95$ or so they charge my card for $100 and try to do another separate charge for $60 or so I immediately called my bank and told them what happened. It was bait and switch. I also emailed this company and told him it was a bait and switch and they were fraud, and they kept on going back-and-forth with me that I want to keep part of. No I want all of my money back it’s fraud Then I received this powdery substance in the mail that I did not order or want. I said it was trying to off me.
