1. The employer is not enforcing social distancing for COVID-19. Employees are working side-by-side. 2. The employer is not enforcing the use of face masks. 3. An employee from first shift (Easy Connect) that had COVID-19, among many other employees that have had the virus, died on
August 27, 2021. It is suspected that he contracted the virus from work.

The employer is not notifying employees if they have been exposed to an employee that has tested positive for COVID-19. The employer is not requiring face masks to be worn, so employees have a greater chance of contracting the virus now.

1. There have been over (40) cases of COVID-19 in the facility during the last two weeks. Employees are being moved from their departments to other departments where there have been positive cases and the employer is not taking the proper measures to protect employees. Employees are
not being notified of the cases until about 1 1/2 weeks later. 2. The employer is supposed to have a cleaning crew coming in to clean and sanitize the building, but a crew has never been seen and there is concern that cleaning is not being done. 3. There are no shields on the tables and employees are working closer than six feet apart. 4. No hand sanitizer is available. 5. No disinfectant spray is available to wipe down work surfaces and equipment.
