The employer failed to follow their IIPP in correcting unsafe or unhealthy conditions, work practices and work procedures in a timely manner based on the severity of the hazard. Electrical hazards have been brought to the attention of the employer and have not been addressed in over 2 years. Title 8 CCR §3203(a)(6) 4. Employers shall provide face coverings and ensure they are worn by employees when required by orders from the CDPH. Face coverings are not provided on a regular basis so they are being reused by employees. Title 8 CCR §3205(c)(6)(B) 5. The employer failed to ensure machinery and equipment are not used or operated under conditions that are contrary to the manufacturer's recommendations or, where such recommendations are not available, the engineered design. Due to the damaged roof, electrical equipment is exposed to water, which is contrary to the manufacturer's recommendations. Title 8 CCR §3328(a)(2) 6. The employer failed to ensure enclosed workspaces, insofar as is practicable, are maintained to prevent the entrance or harborage of insects, rodents or other vermin. If their presence is detected, an effective program of extermination and control shall be instituted. There is a rat infestation, including the employee lunchroom, and no controls have been put in place to ensure effective extermination. Title 8 CCR §3362(f)
