The employer did not maintain an effective injury and illness prevention program in that the employer did not ensure that COVID-19 precautions per the state executive and local public health department orders are being followed at the workplace in the following instances: Description of complaint: The complainant
works for Kaiser Antioch in the PACU department as a patient care tech. Her duties include getting patients from the waiting room in to pre-op, getting them dressed and having them switch into Kaiser supplied PPE. The EE states that patients are making it into outpatient surgery without first knowing the status of the patient. On 2/10/21 alone the EE stated that she was exposed to two positive COVID-19 case who she had taken care of pre/post op. On 2/11/21 the EE found out that these patient were positive for COVID-19 through word of mouth. When the EE confronted her supervisor the supervisor stated that they don't have enough time to tell you every little thing. 1.The employer did not ensure to implement and maintain an effective procedure for screening incoming patients. Ref T8CCR 5199(c)(3)(3). 2.The employer did not develop, implement and maintain an effective procedure to ensure employees are notified of their exposure to a COVID-19 case during the course of their duties. Ref T8CCR 5199(d)(2)(K).

1. The employer did not establish and implement effective methods to decontaminate and/or control contamination of filtering face pieces such as N-95 respirators during the extended use and re-use of them, especially when Health Care Providers are not being cohorted as recommended by the CDC. T8 CCR
5199(e)(1). 2. The employer did not provide effective training and instruction to employees on the introduction of new engineering or work practice controls affecting the employee's occupational exposure or control measures as recommended by the CDC including for the extended use and re-use of disposable respirators on: The risks of Extended Use and Re-use of disposable respirators including the limitations on their use for extended periods and the limits on the number of times the difference types of disposable respirators can be reused; and the administrative and engineering controls to limit potential contamination of N-95 respirator surfaces during extended use and re-use of disposable respirators including: Strict adherence to hand hygiene practices, and Proper donning and doffing techniques to limit contamination to respirator surfaces Proper inspection, storage and disposal of respirators used by HCP on the job during extended use and re-use of respirators. T8 CCR 5199(i).

Procter & Gamble has issued a recall for 8.2 million bags of Tide, Gain, Ace, and Ariel laundry detergent pods due to a risk of serious injury. The packaging of these products can split open, posing a risk if the contents are ingested or come into contact
with skin or eyes. The issue with the packaging was discovered after the company received four reports of children in the United States accessing the liquid laundry packets, three of which reported ingestion during the time period that the recalled lots were sold. However, it is not known if these laundry packets came from the recalled bags. No confirmed cases directly relating to this packaging defect have been reported.
The recall involves Tide Pods, Gain Flings, Ace Pods, and Ariel Pods liquid laundry detergent packets packaged in flexible film bags. These products were manufactured between September 2023 and February 2024. The recall applies to bags containing 12 to 39 laundry detergent packets, with a variety of scents and types:

- Gain Flings Moonlight Breeze Scent
- Gain Flings Blissful Breeze Scent
- Gain Flings Spring Daydream Scent
- Gain Flings Plus Ultra Oxi
