The previous response to UPA 1704096 concerning the bleach bottles was fabricated and to date, there are four bottles of bleach that were not properly stored and mislabeled. 3. All of the institutional fire extinguishers have not been properly serviced and are expired. 4. A working eyewash station for staff or inmates is not available. The portable one on the warehouse wall hasn't been serviced in three years and is full of dirt and bugs.

condition. 2. 29 CFR 1910.134(h)(2)(i) continues to be violated as the employer is not properly storing respirators in a way to protect them from damage and contamination as they are being stored with used and contaminated PPE. The agency has failed to properly keep staff safe from COVID-19 contamination in the C-2 housing unit. Special Housing Unit: 3. Staff are not provided with the proper PPE to keep them safe from COVID-19 such as gowns, face shields, masks and latex gloves. On 12/10/2020, an employee reported pain, redness and a burning sensation from being directly exposed to bleach.

stop any further activity with this entity. I found a phone number for the company Subgenix Products in the bottle. 800-609-4174 and called it. The receptionist answered in a foreign language which sounded oriental and I also heard what sounded like Spanish. When I requested an English speaking person to assist, they hung up the phone. I called again with the same results.
