This complaint disputes the handling of the concerns brought up with previous UPA 1704096 and alleges some new health and safety issues: 1. Donning the PPE in the officer's station is not a safe alternative because it allows staff to come into the area without PPE, thus contaminating the area. There are safer alternatives such as the sallyport area. 2. The previous response to UPA 1704096 concerning the bleach bottles was fabricated and to date, there are four bottles of bleach that were not properly stored and mislabeled. 3. All of the institutional fire extinguishers have not been properly serviced and are expired. 4. A working eyewash station for staff or inmates is not available. The portable one on the warehouse wall hasn't been serviced in three years and is full of dirt and bugs.Alleged Hazards: 4, Employees Exposed: 100
