1. The employer is not following CDC guidelines regarding informing employees of workers who have tested positive for Covid-19. One employee tested positive and the coworkers were not informed until a week later.

2. The employer is not following CDC guidelines for increased cleaning and sanitizing of the facility after employee(s) have tested positive for Covid-19. The employer is not following CDC social distancing guidelines in the kitchen. There are no social distancing measures being taken for the employees working in the kitchen. Employees are required to perform duties in the kitchen that places them within 6 feet of each other.

3. The employer is not following CDC guidelines in regards to the use of facemasks or face coverings. Managers do not wear masks throughout the day. When unmasked the managers are standing directly next to the workers.

4. Employees are not provided appropriate gloves when handling cleaning and sanitizing chemicals. Employees are provided food handling gloves to wear when performing cleaning.

5. There is insufficient quantity of hand sanitizer at the site for employees to use during the frequent use of high contact surfaces such cash registers and door handles.
