Employees are not wearing face masks while prepping the food. Customers are walking in with no masks. A manager saw what happened and did nothing to correct them.
The employer has not implemented elements of an infectious disease preparedness and response plan to protect employees against COVID-19 disease to include identification and isolation of sick employees, including practices for worker self-monitoring and excluding from the workplace any employees with signs and symptoms of COVID-19.
