A COVID-19 pandemic plan has not been developed and implemented for employees to include employee wellness checks, quarantining employees, informing employees for possible exposure to the virus, employee returning to work policy, social distancing, personal protective equipment, disinfecting/sanitizing, hand sanitizer and employee training. 1. T8 CCR § 3205(c)(6) - Employer not providing face coverings to employees for COVID-19 protection in the workplace. 2. T8 CCR § 3205(c)(9) - Employee tested positive for COVID-19 and Employer did not notify staff about it. 3. T8 CCR § 3362(e) - Garbage bins outside... the restaurant are often full with smelly odor. No cleaning done in the area. 4. T8 CCR § 3364(b) - Pipes are clogged. The bathrooms and the area where the utensils are washed have bad odor.

6 individuals at Kentucky Fried Chicken- Firestone in Weld county tested positive for Covid-19. The outbreak began on 3/29/2021
Several employees were exposed to a positive COVID-19 case. The employer tried forcing employees to work rather than letting them get tested. The store was never properly cleaned or sanitized after the exposure.
