1) Charts and records are stored in such a manner as to impede access and egress from office and storage room areas where these documents are kept. 2) The common area where the microwave and water cooler are kept is not maintained in a sanitary condition due
to spilled beverages and/or food debris. 3) Insects such as ants and flies have infested the common area due to the presence of beverage and food wasted. 4) Treatment areas and equipment are not being disinfected following each treatment session. 5) The wearing of facemasks is not enforced in the clinic. 6) Social distancing is not practiced in the clinic.
