1. Employees could be exposed to the COVID-19 virus, as well as other pathogens etc. due to there being only one toilet for employees and patients, and it not being sanitized between patients. 2. Due to the large number of patients coming into the center, employees sometimes
have to wait 20 minutes to use the rest room. 3. There are no barriers between employees and patients, and the premises are cleaned only twice a week, and are not sanitized at all; disinfectant wipes are no longer provided for employees to sanitize their own work areas. 4. When a member of staff was diagnosed, and hospitalized, with COVID-19, the rest of the employees, all of whom had had contact with the affected employee, were not informed.

There is an outbreak of COVID-19 at the ice cream shop and employees are still being required to come into work, including those that are sick. This is exposing other employees to the virus.

Due to being short staffed, employees with symptoms of the Covid-19 virus are required to report to work even if waiting for test results. There is a concern that employees are working while positive for the virus and exposing all other employees and patrons.

1. The employer is not enforcing the wearing of mask. The shuttle operators are required to transport customers in vehicles without Plexiglas to separate the passengers and drivers. 2. Employees are operating a 4 point vehicle lift which lowers without being activated due to a hydraulic leak.

Employees could be exposed to the COVID-19 virus in the workplace, due to: 1. The virus appears to be spreading within the facility: some residents who tested negative for the virus upon arrival have subsequently developed the infection; 2. At least 8 employees have become sick recently,
and other employees, who had or could have contact with those affected, were not informed or tested promptly; 3. Employees, who take their own swabs for their weekly COVID test, might not obtain the swab from far enough up the nose, and could get a negative result when then might be positive.

There has been a COVID-19 outbreak in the facility and there is a concern that the employer is not cleaning properly to reduce exposure. Also the employer is not enforcing the wearing of masks by employees and customers entering the facility.
