The company did not screen those that enter the building and ensure that those The exhibiting symptoms of COVID-19 were separated from other employees and immediately sent home. f. The company continues to permit employees who have recently traveled from other states (Texas and Florida) to enter the office without following CDC guidance and local travel restrictions which requires quarantining for a full seven (7) days for interstate travel. 2. The company did not conduct a Personal Protective Equipment assessment and provide adequate training for workers in the laboratory, workers wear open toe shoes, and fail to wear eye protection and lab coats while working in the lab. 3. Fine particulate is generated in the foundation lab when grinding pigment but there is not adequate capture or collection therefore the particulates suspend in the air exposing nearby workers to a respiratory hazard. 4. There is not eyewash or shower in the mascara and lip labs despite the use of concentrated materials that can cause injury to eyes and skin. 5. The employee refrigerator and water dispenser are contaminated with batch material due to improper hygiene and insufficient cleaning.
