temperature checks on patients. Patients are required to wear masks, but an employee at one point had cut holes in her face mask (several) and wore it like this for weeks. She stated that she couldn't breathe so that was her solution. Employees are not able to socially distance. The office is not cleaned between patients, and office tools are not either.
