1. Employees are exposed to COVID-19 due to the employer requiring employees diagnosed with COVID-19 to continue to work without quarantining as recommended by the Centers for Disease Control (CDC). 2. Employees are exposed to amputation hazards when the light curtain/photo eye installed on machine number 5041 (post fold gluer) have been disengaged. 3. Employees are exposed to caught by hazards when the conveyor system on machine number 3132 (Flx-exo) is not equipped with an emergency stop "wire". 4. Employees are exposed to slip, trip and fall hazards from air line and power cords lying in walkway near the strapper/bundler near machine number 4521 (die cutter). 5.
