1. The employer is not providing employees NIOSH approved N95 respirators . 2. The employer is not conducting employee medical evaluations prior to requiring respirator or N95 use. 3. The employer did not perform adequate qualitative fit testing . 4. Fit testers were not trained resulting in inconsistent fit testing procedures. 5. The employer is not providing respiratory protection training to employees required to wear resiprators. 6. The employer is not providing personal protective equipment (PPE) training to employees. PPE including, but not limited to, gowns, safety glasses, goggles, and faceshields. 7. The employer is providing inadequate and uneffective PPE to employees. PPE including, but not limited to, gowns, safety glasses, goggles, and faceshields. 8. The employer is not providing Hazard Communication training to employees required to utilize disinfectant cleansers for PPE decontamination.
