1. Lockout/Tagout program is not provided for employees conducting maintenance and servicing on equipment such as, but not limited to wood chippers, directional drill rigs or VAC's. 2. A 55 gallon drum filled with brake cleaning solvent is stored directly under an electrical outlet located at the third bay door. The drum is not covered. 3. The legs for the gantry crane in the Shop Area are bent. The crane could collapse. 4. The hoist for the gantry crane located in the Shop Area has never been inspected. 5. The forklift has not been inspected prior to use. 6. Compress gas cylinders are not secured in shop area. The cylinders could fall over. 7. There is hot water leaking from the handle of the steam pressure washer located at the front of the building. The hot water is causing burns to employee's hands. 8. The locking pawl mechanism are damaged on four floor jack stands in the Shop Area. 9. Cans of spray paint is not stored in a flammable cabinet. 10. Parts are stored in front of the electrical panel box located at the rear entrance of the Shop Area. 11. A COVID-19 pandemic plan has not been developed and implemented for employees to include employee wellness checks, quarantining employees, informing employees for possible exposure to the virus, employee returning to work, social distancing, personal protective equipment, disinfecting/sanitizing, and employee training.
