1. Employees are exposed to trench collapse hazards while working in trenches. The trenches are over 3 feet deep and trench boxes are not being used. 2. Employees are not wearing personal protection equipment such as safety shoes, safety hats and high visibility vests. 3. Employees are exposed to overhead hazards caused by the use of nonrated lift chains, which have bent links. 4.
