'- Mold in Mulberry, Ivy, and Lake units. - Strong mildew odor throughout bedrooms and younger boys dayroom. - Toilet coming up through shower. - Ratio of staff to students within the units has led to injury to the staff. - Water is coming out of faucets brown to the point a grudge is left on the sink. - Dirty mop heards are in cleaning closets. - Sufficient cleaning supplies are not supplied to staff such as Lysol. - No sanitization of units. - Staff who tested positive for COVID-19 are allowed back on the premises without testing negative. - No PPE for staff.Source: Osha.gov | Receipt Date: 2020-11-21
