Covid outbreak over the past few weeks. Lack of proper PPE and surgical masks for staff. Lack of clean mop heads and near empty chemical dispensers and unit supplies are inadequate. Staff, who tested positive, are allowed to return after one week with no retest required.

Over 100 employees are at risk of contacting covid 19 due to the improper screen and not enough PPE. The thermometers are reading at 98.0 for everyone.the nurse admitted that they don't work. They also are not sanitizing them. There is no hand sanitizer nor mask provided.

There has been 6 confirmed cases at the main center and employees are being forced to continue working in different units instead of social distancing. Employees have also been attending school with kids from the main center and the group home. There has been no notification of these cases as well they are keeping it from staff as if it's a secret. They also have yet to test the children even after having confirmed cases.

Students are going out of state to OH and different cities in IN on home visits And not being quarantined nor tested upon returning. Everyone including the kids are at high risk for covid due to improper screening.
