On December 2nd a team lead came to me and told me to fill out the covid vaccine status form. When the device I was filling it out on crashed multiple times, the team lead told me to just falsify my response instead of finding a way to fill out the form honestly. There was one other employee present, but I feel that this action put the entire store at risk.Alleged Hazards: 1, Employees Exposed: 1
