1)No Corona testing. Complainant was employed from Feb- April 2021. My temperature was taken on my 1st day and that's it. 2)There are no masks made available for clients or staff. They put a box here an there, no consistancy. At least 30 employees exposed. 3)The door an window downstairs leaks water all over the floor when it rains. There's 1,maybe 2 on staff for housekeeping. Complainant cleaned the beginning of every shift. You'll never smell cleaning going on there, dirt so thick when Complainant sprayed Clorox dirt literally ran down the walls, chairs, computers etc. At least 75 staff an clients. 4)They have 2 out side buildings. No lighting, this winter when it iced so bad, complainant fell. The ice was several inches thick. They scrape the lot but leave it in piles. Complainant didn't tell anyone because complainant had no idea who to tell. Complainant was exposed in falling. But the lot and walk way to buildings is exposure for all. 5)The cafeteria, is disgusting. Complainant saw mold on bread and cheese. Mice are common. All on site are exposed at least 75. 6)Techs are allowed to transport and dose patients. This can effect 1-9 people depending on how many are in PHP
