1. MIOSHA COVID 19 guidelines for Outpatient Healthcare Facilities are not being followed. 2. There is not a COVID 19 Preparedness & Response Plan. 3. There are no recordkeeping practices about employee temperatures or screening. 4. Employees are allowed to come into the building through doors that do not have a screener. 5. There are multiple doors that employees enter and exit, which can lead to an employee being in the building unscreened for periods of time. 6. The screening for employees does not include a questionnaire, rather just having their temperatures taken. 7. Some meetings held have more than 10 people in attendance and employees are sitting closer than 6ft to another person. 8. The office had a positive COVID case and that person wasn't screened prior to being in a room with people (closer than six feet, not all wearing masks, and eating) which then found their temperature to be elevated. 9. The office was not closed in order to ventilate or clean after the employee left the office. 10. Only one other employee was instructed to quarantine.Source: Osha.gov | Receipt Date: 2020-09-11
