1. An electrical cabinet located in the wastewater department is bypassed to reset a breaker that trips. 2. An office is set up in an waste treat electrical room meant for authorized personnel that does not maintain 3 feet of clearance from working panels. 3. Employees are exposed to electric shock from exposed wiring, water leaking on active control switches, permanently daisy chained extension cords in the waste treat area. Daisy chained cords power heaters that are used to keep sludge tanks warm. 4. Electrical panels are opened and bypassed despite arc flash warnings. 5. Chemical totes are exposed to high speed forklift traffic. Employees must make turns near totes containing polymers and coagulants, and forklifts have difficulty climbing a ramp while carrying totes of sulfuric acid and caustic. 6. Employees are exposed to high levels of hydrogen sulfide (H2S) without adequate personal protective equipment or gas detection equipment. 7. Employer is not enforcing COVID-19 protections including not enforcing 6 feet of distancing, wearing of face masks, and conducting employee temperature checks with a functional thermometer.
