Item# 1: Nurses in the Emergency Department, ICU and COVID-ICU units are not provided access to additional respirators throughout their shift. Item #2: Nurses in the Emergency Department, ICU and COVID-ICU units are not provided access to additional gowns throughout their shift. Item #3: At entry points the screening and triage of visitors is not consistent, visitors are allowed to enter the facility without being asked their COVID-19 status and/ or while exhibiting symptoms. Item #4: In the Emergency Department, the employer is not enforcing employees use of facemasks and physical distancing is not maintain; in that, access to the emergency room waiting area is not limited at any given time there are about 50 people in close proximity. Item #5: The COVID-19 plan and hazard assessment did not include input from non-managerial employees and their representatives.
