The employer did not identify, evaluate, and/or correct hazards from COVID-19 in the workplace by following the public health order by: - Cleaning and sanitizing shared work areas and high-touch surfaces, particularly after employees tested positive for COVID-19; - Implementing barriers to protect employees from customers or each other, where feasible, such as by cashiers or at drive-thru windows; - Requiring employees and customers to wear face coverings; - Providing training to employees on the hazard of COVID-19 in the workplace, how to properly wash their hands and wear a face covering, the measures that the employer has taken to protect employees, and measures the employees can take to protect themselves.
