1. The employer provides 40 restrooms in the facility and at least 26 of them are not cleaned on a regular basis. There are feces and urine on the toilet seats and walls. There are bugs crawling in the restrooms. Trash cans are overflowing. 2. The employer is no longer conducting the additional cleaning protocols implemented to control the spread of COVID-19, such as wiping down handrails and walls, cleaning dividers in the break rooms, wiping down tables in the break rooms, and emptying trash cans.Alleged Hazards: 2, Employees Exposed: 1600
