1. Employees are exposed to struck-by hazards, in that the employer does not provide powered industrial truck training to employees who operate forklifts. Location: Throughout the facility 2. Employees are exposed to fire hazards, in that employees are allowed to smoke while in close proximity to flammable gas on powered industrial trucks. Location: Throughout the facility 3. Employees are exposed to electrical hazards, in that the #5 press machine and uncoiler have spliced power cords covered with electrical tape and wrong sized replacement cords. Location: Throughout the facility 4. Employees are exposed to electric hazards, in that there are missing electrical panels and exposed electrical wires on press machines and control panels. Location: Throughout the facility 5. employees are exposed to electric hazards, in that the employer has not developed and implemented control of hazardous energy procedures for employees performing servicing and maintenance on electrical equipment, including but not limited to, press machines and uncoilers.
