1. Eye wash station location in sink where dirty instruments are placed. The eye wash station is not properly marked. 2. Lack of proper sterilization of dental handpieces, sterilized in bags and then bags are open and "sterile" handpieces are stored on an open counter for prophylaxis. 3. Handpieces are reused and not sterilized between patients. 4. Employees don't know where to find hazardous material forms for chemical exposures when in the office and cannot access them outside the office either. 5. Management improperly wearing masks. Masks sometimes not covering nose or mouth, touching face or mask and then touching patient. 6. No clearly outlined exposure plan for Blood Borne Pathogens readily available to employees.Alleged Hazards: 1, Employees Exposed: 1
