1. Four office employees tested tested positive for COVID-19 during the first week of October. Employees found out about other employees being positive from their coworkers as opposed to finding out from the employer. 2. The employer did not conduct any contact tracing. 3. Prior to the outbreak, the use of masks was not being enforced in the office. 4. After the first person tested positive for COVID-19, the employer did not do a deep cleaning of the office to disinfect properly.Alleged Hazards: 4, Employees Exposed: 60
