'-On-the-floor and in-office management teams not enforcing proper mask-wearing regulations (such as masks being worn below the nose; having signs posted in the building about certain restricted types of masks i.e. handkerchiefs, bandannas that were still being worn anyway; plastic face shields being worn without any sort of secondary mask worn underneath; issues with masks being removed while talking or periodically throughout the day also not addressed or enforced). - Management team not enforcing social distancing regulations. - Disinfection of process area recommended but not enforced. - Employee who was sick at her station continued to work at her station. Vomit was not cleaned up immediately as it was instead covered up with a felt pad.
