Work place is not routinely cleaned and disinfected throughout the day. An employee recently tested positive for COVID-19 and employees who worked with the employee and/or ate in the break room with this employee, were not informed of required testing, quarantining, and return to work CDC guidelines. A dentist was hospitalized and passed away from COVID-19 in January. Employees that worked in close contact with the dentist were not informed of the required testing, quarantining, and return to work CDC guidelines.Alleged Hazards: 3, Employees Exposed: 10
