Regulatory Report

1055 Medical Park Drive Southeast, Grand Rapids charter Township, 49546 Michigan, United States

1. An employee was informed by a co-worker, not management, about an adolescent patient that tested positive for COVID-19 on Monday, October 12th. The company has not alerted staff of the exposure, an employee only became aware after receiving a texted copy (provided from another staff member) of the memo that had been posted in the building. Over the weekend there were seven (7) employees on first and second shift and four (4) on third shift that were potential exposed. Up until this memo being posted, unknown patients have been quarantined while waiting for test results to come back. Employees who had worked over the weekend have not been formally notified of the exposure. A few staff members also worked on other units after working for 8-hours on the unit with the covid positive patient. 2. The employer is now requiring that N-95 respirators be worn on the unit for the next two (2) weeks and there has been no fit testing done. 3. Patients are encouraged to wear masks and most rarely do, and social distancing is not enforced in common areas. 4. Employees have not been given instructions for isolation or covid testing.
