1. The employer failed to ensure periodic cleaning of the facility in accordance to CDC (Center for Disease Control and Prevention) guidelines for the COVID-19 pandemic. 2. The employer failed to inform employees that they have worked with employee(s) who were sick with COVID-19 (coronavirus disease) within the facility as in accordance to CDC (Center for Disease Control and Prevention) guidelines. 3. Employees are exposed to health hazards associated with COVID-19 in that the employer does not screen employees for symptoms of the virus per CDC (Center for Disease Control and Prevention) guidelines. 4.
