1. The kitchen floor is wet due to a leaky dishwasher, exposing employees to slip/trip/fall hazards. 2. The roof leaks above the bar and by the bathroom, exposing employees to slip/trip/fall hazards. 3. The employer is not implementing adequate safety practices to protect its employees from the spread of SARS-CoV-2 such as, but not limited to: a) Screening employee temperature and assessing symptoms prior to starting work. b) Providing facemasks to all employees for use while at work and requiring their wear. d) Increasing the frequency of cleaning commonly touched surfaces. e) Requiring employees that are symptomatic or positive for COVID-19 to quarantine and not enter to the workplace. 4.
