1.) Walking-working surfaces in the developer room are not kept clear of standing water which presents a slip/trip/fall hazard. 2.) The employer does not have a continuous and effective extermination program to prevent the entrance or harborage of vermin in the establishment, such as, but not limited to, cockroaches. 3.) Employees have not received training on hazardous chemicals (aka Hazard Communication), bloodborne pathogens, or the use of x-ray equipment. 4.) The employer is not implementing precautions to protect employees from contracting and spreading the Coronavirus (COVID-19) in the workplace: a) The wearing of face masks/coverings by employees is not enforced; b) Social distancing among employees and other persons (i.e., patients) is not enforced; c) Common areas and frequently touched surfaces are not regularly cleaned and disinfected; d) Employees known or suspected to be infected with COVID are allowed to report to and remain at work; e) After an employee tested positive, contractors/employers who had employees onsite were not notified of the exposure within 24 hours; f) After an employee tested positive, additional cleaning or disinfection did not occur. g) When notifying employees of COVID positive employees, the employer did not keep the identity of the positive employees confidential; and h) Employees have not been trained in accordance with 16VAC25-220.
