1. Covid screening questions and temperature checks of clients or employees are not being recorded. 2. High touch surfaces are not being disinfected properly, insufficient contact time with the disinfectant and the surface being disinfected. 3. Insufficient and inadequate personal protective equipment (PPE) is used within the clinic. 4. Patients waiting in the lobby area are not practicing social distancing and commonly sit within three feet of each other. 5. No plastic shields at the check in / check out window putting the employees at risk. 6. Candy bowl and pen cup located on counter are accessible by patients creating a high touch surface. Clients sign-in on the same page as other patients creating a potential covid exposure point. 7.
