1. There is exposed Gears, sprockets, and chain drives exposed to contact on the inner left and right sides to a JNL MARK 5 specialty folding machine. Delivery section in Compressor Machine area. 2. The personal protective equipment assessment is incorrect. 3. Improper care or machines / maintenance and unsafe practices that are taught / employees are being trained to ignore guards / specific rules while setting up jnl lines / general machines / ward 1 and 2 and baysik machine. 4. An incident was reported to HR last week for a 2nd shift supervisor and an employee being inside a jnl mark 5 machine while it was actively running. The operator has stopped the machine 3 times when hands were working on the active line / Demanded that all personel leave the inside of the machine so production can either start or fixes to set up could take place. the 2nd shift supervisor became upset and angry with the machine operator and demanded he shut up and keep the machine runing. causing more aggression towards the operator. 5. There was an accident recorded for JNL2 mark 5 on shift 2. Employees were told to set up a box to be ran on jnl2. During set up employees are trained to jog some boxes through the JNL line and ignore the unguarded belts and pulleys, rollers and rails during this stage of the set up. Folding orders require special attention in order for a proper set up. There is no set up instructions. No listing of tools / machanical parts to help aide in the instructions of setup for the operator or crew on the machine. 6. Covid19 regulations are not followed or enforced.
