T8CCR 3380(f)(1)(A) - The employer failed to assess the workplace to determine, select and have each affected employee use personal protective equipment (PPE) for any hazards that are present which necessitate the use of PPE T8CCR 3205(c)(6)(A) - The employer failed to insure that employees are separated from each other and from other persons, by a distance of at least six feet, in all indoor work areas and in the Lunch and Break Rooms T8CCR 3205 (c)(7)(A) - The employer failed to provide clean and effective face coverings, including prohibiting the use of paper napkins as a substitute for face coverings. In addition, the employer failed to ensure face coverings were worn by employees at all times over both their nose and mouth and while employees use the bathroom. T8CCR 3205(c)(10) - The employer failed to prevent the transmission of COVID-19 by not ensuring that identified Positive COVID-19 cases in the Q-C Department, were excluded from the workplace, until the Return To Work requirements of subsection (c)(11) were met. Additionally, the employer failed to exclude the employees with likely exposure to Positive COVID-19 cases, from the workplace for 14 days after the last known date T8CCR 3205(c)(2)(B) - The employer failed to develop and implement a screening process for employees to detect the COVID-19 symptoms, such as asking employees to self-evaluate their own symptoms before reporting to work and training employees on how to properly take temperature measurements with non-contact thermometers.
