1. The employer failed to give notice of the potential COVID-19 exposure, within one business day, to all employees who may have had COVID-19 exposure and their authorized representatives. The employees were not informed by the employer of the exposure. They were made aware through word of mouth by other employees. Title 8 CCR § 3205(c)(3)(B)3. 2. The employer failed to ensure face coverings are worn by employees over the nose and mouth when indoors, when outdoors and less than six feet away from another person, and where required by orders from the CDPH or local health department. Wearing face coverings is optional when working in the office and for field carpenters when outdoors and less than six feet away from each other. Title 8 CCR § 3205(c)(7)(A) 3. The employer failed to ensure that COVID-19 cases are excluded from the workplace until the return to work requirements of subsection (c)(11) are met. Employees that tested positive for COVID-19 returned to work after 3 days of quarantine. Title 8 CCR § 3205(c)(10)(A) 4. The employer failed to exclude employees with COVID-19 exposure from the workplace for 10 days after the last known COVID-19 exposure to a COVID-19 case. The employer encouraged employees that had COVID-19 exposure to return to work as soon as possible, not requiring them to quarantine for the 10 days. Title 8 CCR § 3205(c)(10)(B)
