1. There have been more than 30 COVID-19 positive cases, but the company has not notifying the local health department of the positive cases so contract tracing can be done. 2. Employees are allowed to work with symptoms and not quarantine as required. 3. There is a lack of housekeeping/sanitation being done in the facility, so it is a serious hazard with all of the used supplies and trash laying around on the floors and counters. 4.
