Not cleaning regularly to prevent exposure, this includes parts everyone touches and restrooms. 3. Social distancing not established, and staff work in close range with one another. 4. Not quarantining employees who have symptoms and test positive. 5. Not informing employees of positive COVID cases at the facility.
