1. On or about April 14, 2021, an employee notified the employer that they may have COVID-19 and wanted to stay at home. The employee was told to come to work or be fired. The employee found out while at work that they were positive for the virus after being in close contact with other employees in the facility, work vehicle and on jobsites. This exposed the other employees that were working in direct contact with the employee. 2. Employees that were exposed to the employee who was positive for COVID-19 were not notified so that they could quarantine and be tested for the virus. 3.
