1 . On or about 2 Mar 2021, Asbestos removers and demolition workers were not provided PPE required to perform task associated with this type of labor. 2. On or about 2 Mar 2021, Materials/Debris from upper floors of site are incorrectly sent down holes in each floor creating overhead hazards from debris onto workers on respective floor below. 3. On or about 2 Mar 2021, Housekeeping is not conducted as to mitigate excessive dust on each floor of worksite 4. On or about 2 Mar 2021, Workers are exposed to fall hazards as holes in floors are not correctly labeled and covered during workday. 5. On or about 2 Mar 2021 , Covid 19 Protocols are not being enforced as employees are not wearing mask and employees are not required to maintain social distancing protocols.
