1) Many powered industrial truck (PIT) operators have not received certified safety training. These include a riding & standing forklift, several power jacks and about 12 cherry picker units. Operators of cherry pickers often elevate themselves to retrieve orders without safety lanyard use for fall protection. 2) Tight spacing of merchandise in storage causes order pickers to frequently climb on/in racks. Some racks are wobbly due to loose assembly. 3) Rolling stairs used by warehouse employees are unsafe due to broken structural members. 4) Surfaces of equipment such as PITs, which employees contact / share are not being regularly disinfected as part of the company's COVID-19 infection prevention program.Alleged Hazards: 4, Employees Exposed: 35
