Regulatory Report
900 South Beacon Boulevard, Grand Haven, 49417 Michigan, United States
1. Since the beginning of the pandemic no meetings have been held regarding: Covid-19, PPE or the facility plan for suspected or confirmed cases. The only education that was given on PPE and Covid-19 were from posters and pamphlets hung on the wall. 2. There were no isolation wing or rooms established until the facility received its first positive patient on May 6, 2020. On May 7, 2020 four (4) cases of Covid-19 were confirmed. Not all the employees were notified of the positive cases. Employees that worked with the positive Covid-19 residents were not required to self-isolate or be tested. 3. CDC states that once there is a confirmed case in the facility all health care providers should wear all required PPE for all residents regardless of symptoms on affected unit or facility wide. Only employees working in the new Covid-19 wing get face shields and gowns. PPE should be provided to all four halls instead of just the lock down unit. 4. New residents to the facility have been mixed in with long term residents without being isolated beforehand and are not put on any precautions. 5. When residents are placed on precautions staff is not given face shields, just gowns & are required to wear the same mask that is worn for care of all the other patients. 6. They are only giving N95 masks to staff that are working in the new Covid-19 wing. N95 masks are supposed to be fitted to the face of the wearer and require the wearer to have completed training, this has not been done. 7. When staff member exhibit sign and symptoms of Covid-19 they are not being tested. Management says they can come back to work after two to three days, not 14 days, which is crazy and puts everyone at risk. 8. Employees are provided with disposable masks and are required to wear the same mask for a week. After a day the masks are disgusting but we are told to re-wear them, they are still usable.Source: Osha.gov | Receipt Date: 2020-05-08
