1. Current mitigation steps for COVID-19 including, masks being provided for employees and visitors, meetings being conducted online and not in person and not returning to work if testing positive for the virus without a negative test, are not being strictly enforced. 2. The employer does not provide face masks for employees. Employees must supply their own masks. 3. When an employee tests positive for COVID-19, the employer does not notify employees. They have to find out through "word of mouth" and are not aware if they were potentially exposed to the virus. 4. There is an inconsistency of the COVID-19 testing policy. Some employees who were exposed to the virus were told by management to be tested and then come back to work without quarantining. Other employees have been told to quarantine until they received their test results. 5. After incidents of positive COVID-19 tests, common areas where the infections occurred were not cleaned or disinfected. This includes the laboratory, control room and lunch room. 6. Workers were moved to different shifts to cover for employees that were out with COVID-19 and several of the employees were exposed and infected with the virus due to areas not being cleaned. 7. CPR training has been conducted in the conference room with 20 people in the room and some employees not wearing face masks. 8. The break/lunch room is a small room that is not regularly disinfected. It is shared by all hourly employees and there is no limit to the number of people in the room at any one time. 9. The control room is a small room with employees running production. There is little to no air flow or ventilation and no regular disinfecting is done after COVID-19 cases.
